AD&MA Healthcare Private Limited (“us”, “we”, or “our”) operates a website www.zurihealthcare.com and mobile application (hereinafter referred to as “Service”).

    1. We”, “Our”, and “Us” means and refer to the creators of this privacy policy.
    2. You”, “Your”, “Yourself” and “User” shall mean and refer to natural and legal individuals who use the Website/Application, including users, patients, general Users and Practitioners.
    3. Information” shall mean and refer to any personally identifiable information that We may collect from You. 
    4. Third Parties” refer to any Website/Application, company or individual apart from the User and the creator of the Website/Application. 
    5. Practitioners” means any healthcare provider or professional, including but not limited to doctor, therapist, counsellor, nutritionist, dermatologist, nurses and other medical professionals.

    1. You acknowledge that this Privacy Policy is a part of the Terms of the Website and the Services. By accessing the Website or Application or by otherwise providing Us Your personal Information Yourself or through a primary user or by making use of the Services provided by the Website or Website/Application, You unconditionally signify Your (i) assent to the Privacy Policy, and (ii) consent to the utilization of your personal Information in accordance with the provisions of this Privacy Policy.
    2. You acknowledge that You are providing Your personal Information out of Your free will. If You use the Services on behalf of someone else (including but not limited to, Your child – minor or major or as a legal representative of an individual with mental illness) or an entity (such as Your employer), You represent that You are authorized by such individual or entity to (i) accept this Privacy Policy on such individual’s or entity’s behalf, and (ii) consent on behalf of such individual or entity to Our collection, use and disclosure of such individual’s or entity’s personal Information as described in this Privacy Policy. You hereby consent that the utilization of any Personal Information in accordance with the provisions of this Privacy Policy shall not cause any wrongful loss to You.

You consent to having your past consultation information (including prescriptions, scans, and instructions given by a doctor you previously consulted on the Zuri Website/Application) accessed by doctors who you have booked a consultation with so that they get a comprehensive understanding of your medical history and can advise you accordingly based on previous diagnoses, medications, health conditions, allergies, etc.
    4. YOU HAVE THE OPTION NOT TO PROVIDE US THE PERSONAL INFORMATION SOUGHT TO BE COLLECTED. YOU WILL ALSO HAVE AN OPTION TO WITHDRAW YOUR CONSENT AT ANY POINT, PROVIDED SUCH WITHDRAWAL OF THE CONSENT IS INTIMATED TO US IN WRITING. If You do not provide Us Your personal Information or if You withdraw the consent to provide Us Your personal Information at any point in time, We shall have the option not to fulfill the purposes for which the said personal Information was sought and We may restrict You from using the Website/Application or Services.
    5. Our Website/Application are not directed at children and minors and We do not knowingly collect any personal Information from children and minors.

    1. In order to use the Services of the Website/Application, You are required to register Yourself by verifying the authorized device. This Privacy Policy applies to your Information that we collect and receive on and through the Website/ Application.

    3. Non-Personal Information such as information that cannot be used to identify you such as the webpages that you have viewed and your usage patterns, including certain types of device information.
    4. Having regard to the foregoing, below is an inclusive list of Information that we collect from you in connection with your access and use of the Website: 
    5. Information that we need you to provide to enable you to sign-up for the use of the Website provided by the Company.
    6. Account Information: When you sign-up for an account on the Website (“User Account”), we require certain Information such as your name, phone number and email address for the purpose of user registration, identification and account verification.
    7. Medical Information: If you use the Website to book appointments to consult RPM, you will be providing us and the respective RPM with the relevant Information pertaining to your health conditions and well-being and other medical or health information. Details about the User’s patient’s name, age (optional), sex, contact number, patient ID (as may be maintained by the User), date of visit to the clinic, past and present diseases, allergies, treatments, medical history, diagnosis reports, prescriptions and any images of any case sheet containing medical information, or images pertaining to the patient’s reports and prescriptions uploaded by the User, or such other information as may be uploaded / published voluntarily by the User; and Information pertaining to the User’s computer and web browser, IP address and device ID.
    8. Payment Transaction Information: We may collect your first name, last name, phone number, and billing address, when you register for availing the services offered on the Website. Your payment details are collected through a third-party gateway to which you will be required to provide requisite information separately in order to process your payments. 
    9. Information we automatically collect from your use of the Website: When you use the Website, we automatically collect information, about the functionalities you use on the Website and how you use them. This Information is necessary for the effective performance of the contract between you and us, to enable us to comply with legal obligations given our legitimate interest in being able to provide and improve the functionalities relating to the Website and the Services.
    10. Geo-location Information: When you use certain features of the Website, we may collect information about your location such as your IP address, which in turn would enable us to infer your city, region/state, country, continent, internet service provider (ISP), longitude/latitude coordinates of location, type of connection (like residential, business, etc.), and name of the organisation that owns the connection (i.e. if it is a business connection or not).
    11. Usage Information: We collect Information about your interactions with the Website such as the pages viewed on the Website and the User Account dashboard, content created or uploaded through use of the Website and Services.

This includes details of how you used the Website, such as access to your User Account, certain information that your web browser sends whenever you visit the Website, internet protocol address, information inferred from your internet protocol address like location, ISP details, device information such as hardware configuration and operating system, browser types, referring / exit pages and URL’s, number of clicks, domain names, landing pages, pages viewed and other such information. We do not link this automatically collected data to Personally Identifiable Information.
    13. Cookies and Tracking Technologies: We collect device specific information (such as your hardware model, operating system version, unique device identifiers). We may also associate your device identifiers with your User Account. You may decline these cookies by changing your browser settings, if permitted. However, do note that this may impact certain features of the Website or your experience in using the Website.
    14. For Practitioners or Providers using the Website/Application, we would collect your name, education, specialization, address of practicing hospitals/clinics, contact information, achievements,) Location, etc.

Information we collect from third parties: We may work with a limited number of third parties (including mobile network providers, and delivery service providers, collection agencies, analytics providers) to provide the Services and may receive information about you from such third parties. We also receive profile information from the social media platform(s) that you use to log in or create accounts with us, as per their terms and policies.
    16. All the Information provided to the Company by you, including Personally Identifiable Information or any Sensitive Personal Information, is voluntary. You understand that the Company may use certain information of yours as provided under this Privacy Policy.
    17. This Privacy Policy does not apply to, nor does the Company take any responsibility for any Information that is collected by any third-party, either using the Website or through any third-party links made available to you during your use of the Website. 
  5. HOW WE COLLECT PERSONAL INFORMATION
    The methods by which We collect Your personal Information include but are not limited to the following:
    1. When You register on our Website/Application, When You provide Your personal Information to Us during the course of Services provided to You by Us;
    2. When You use the features on Our Website or Website/Application, Privileged & Confidential Through Your device, once You have granted permissions to Our Website/Application;
    3. Through HSP pursuant to consultation on the Website/Application, By the use of cookies, We collect information that Your browser/app sends whenever You visit Our Website/Application, such as, the log data. 
    4. Through surveys and/or events conducted during the course of Services provided to You by Us.
    5. We may use third party services that collect, monitor and analyze this. This information is kept completely secure. 
  6. OUR USE OF YOUR INFORMATION
    We will use the information collected from you for the following purposes: 
    1. Ensuring the Services with respect to the Website is presented in the most effective manner for your use which includes providing RMP details about you to the extent necessary for their consultation services and as consented by you;
    2. To improve and secure the Website: We are always looking for ways to make our Website smarter, faster, secure, integrated, and useful to you. We use collective learnings about how Users use the Website and feedback provided directly to us to troubleshoot and to identify trends, usage, activity patterns and areas for integration and improvement of the Website;
    3. To provide you with information pertaining to the Services we offer, send newsletters and transactional and user support and Services that you request from us;
    4. To carry our obligations arising from any contracts entered into between you and us, including for billing;
    5. To notify you about any changes and improvements to the Website; or analytical purposes, including but not limited to assessing usage data, usage patterns, and other similar activities;
    6. When you create a User Account, we keep a record of your communication to help solve any issues you might be facing;
    7. In any other way that we may describe to you at the time you provide the Information or to fulfil any purpose for which you provide the Information;
    8. We use Information collected from cookies and other tracking technologies to improve the User experience.
    9. To provide and process service requests initiated by you.
    10. To validate, authorize and map a specific profile with an authorized User.

    12. The information made available by patients is displayed to Practitioners and vice versa. The information collected from Practitioners is displayed on the Website/Application to enable a User to connect with a Practitioner of their choice.
  7. PERMISSIONS
    Once You download and install Our Website/Application, You may be prompted to grant certain permissions to allow the Website/Application to perform certain actions on Your device. These actions include permission to:
    1. read/write/modify/delete data in relation to the Website/Application on Your device’s storage;
    2. view/access information relating to networks/access networks, including permission to send and receive data through such networks/access networks;
    3. determine Your approximate location from sources like, but not limited to, mobile towers and connected Wi-Fi networks;
    4. determine Your exact location from sources such as, but not limited to, GPS;
    5. view/access device information, including but not limited to the model number, IMEI number, operating system information and phone number of Your device;
    6. access device information including device identification number required to send notification/push notifications.
  8. DISCLOSURE OF YOUR INFORMATION TO THIRD PARTIES
    1. When you use our Website/Application, we must provide some of your personal Information to third parties to give you better services and for enhancement and visibility of our Website/Application.
    2. Due to the existing regulatory environment, we cannot ensure that all of your Information shall never be disclosed in ways other than those described in this Privacy Policy. For example, but without limiting and foregoing, we may be obligated to disclose Your Information to the government, law enforcement agencies or other Third Parties. Under certain circumstances, Third Parties may unlawfully intercept or access transmission or private communications, or abuse or misuse Your Information that they may collect from our Website/Application. Therefore, we do not promise, and you should not expect, that your personally identifiable information or private communications would always remain private. We may share your personal Information to those service providers and Practitioners with which you make arrangements through our app. We provide them with the Personal Information needed to make and complete the appointment / answer the query you pose.
    3. We may share our data, including Your Information, with our parent and/or subsidiaries that are committed to serving your needs through use of our Website/Application and related services, throughout the world.
    4. We cooperate with law enforcement inquiries, as well as other third parties to enforce laws, such as: intellectual property rights, fraud and other rights. We can, and You so authorize Us, disclose Your Personal Information to law enforcement and other government officials as We, in Our sole discretion, believe necessary or appropriate, in connection with an investigation of fraud, intellectual property infringements, or other activity that is illegal or may expose Us/ Us or You to any legal liability.

    This Agreement has been executed and delivered in India, and its interpretations, validity and performance shall be construed and enforced in accordance with the laws of India, and shall be subject to the jurisdiction of the COURTS IN MUMBAI.
